Certified copies of Coos County birth and death certificates can be obtained from Coos Health & Wellness, Office of Vital Records, for six (6) months following the event. They may be obtained either in person in our office, or requested through the mail. An order form and photo I.D. is required regardless if the certificate is ordered in person or via the mail.
DEBIT/CREDIT TRANSACTION FOR AN EVENT WITHIN THE LAST SIX MONTHS:
Payment may be made by check, cash, or money order payable to Coos Health & Wellness. May also pay by debit/credit card online or in office.
PLEASE NOTE:
- If paying by check, it may be up to 15 days from receipt of payment before the vital records are issued.
- If paying by credit/debit card there is a 2.49% or a minimum $2.00 convenience fee.
- Point and Pay (not Coos County) will appear as the merchant of record next to your payment on your bank account or credit card statement. Coos County does not collect or retain any portion of the convenience fee charged.
Birth Certificate
To request a Birth Certificate, an order form must be filled out and submitted for either certificate issuance.
Please include the following information in your request:
- Birth name, date and place of birth
- Father’s name and mother’s maiden name
- Your relationship to the person of birth
- Reason for request
- Mailing address
- Copy of photo ID
- Fee $25.00 per certified copy

- Your relationship to the decedent *restrictions apply for individual ordering eligibility
- Reason for request
- Mailing address
- Copy of photo ID

After six months, the birth and death records are transferred to the State and need to be obtained through the:
