The older adult program is for individuals who are at least 60 years old and receiving services at CHW. The older adult program offers therapy, case management, skills training, Enhanced Care Outreach Services (ECOS) and Pre-Admission Screening and Resident Review (PASRR). We work closely with Aging and People with disabilities and the local assisted living/nursing facilities.
ECOS is designed for individuals who need wrap-around mental health supports to live successfully in the community. Treatment provided is focused on teaching skills and coping strategies to enable individuals to live with greater autonomy in the least restrictive settings. The program is able to provide individual counseling, skill training, case management, consultations and medications management for the individual.
