Assertive Community Treatment is the evidenced based practice model for achieving good outcomes for the highest needs/ highest acuity adults with severe psychotic and mood disorders. ACT uses a multi-disciplinary team “hospital without walls” approach with very low caseloads to achieve success using many of the principles of Wraparound teams.

Team disciplines include a Master’s level Clinical lead who provides oversight, runs the daily “huddle” meeting, and delivers direct services; a Substance Abuse specialist who delivers both individual and group Enhanced Illness and Management Recovery (E-IMR) interventions; a Vocational/Supported Employment specialist; an RN who is mobile and assists with both psychiatric as well as medical needs; an LMP; a peer support specialist; and other team members to do skills coaching, case management and housing support.
