CaCoon | Coos Health & Wellness

CaCoon is a program for children with special health care needs, ages birth through 20 years of age, and their parents/caregivers. Children and youth with special health needs are those who have or are at risk for having a chronic physical, developmental, behavioral, or emotional condition. One in five children in Oregon fits this description.

CaCoon stands for Care Coordination, and services are provided by public health nurses to support your child and your family. Your CaCoon nurse can help you locate, connect with, and coordinate with needed resources and services to better meet you and your child’s needs. Your nurse can also monitor your child’s growth and development, and suggest ways to help you with your child’s developing needs.

Eligibility

  • Children and youth ages birth – through 20 years of age with special health needs with Oregon Health plan coverage
  • Parents/caregivers of enrolled children with Oregon Health plan coverage
